At Sunrise, our Case Manager is responsible for managing the customer’s health and wellness as indicated by the Service Care Plan. This will include the supervision of Home Health Aides and other Health Professionals, communication with the customer’s family members and physician, regulatory compliance and quality improvement. The Case Manager is responsible for performing all job requirements in a manner that exemplifies the Dignity Home Care core values.

RESPONSIBILITIES & QUALIFICATIONS

Responsibilities:
- Participate in the establishment of services and procedures
- Provide in Home Supervision of Home Health Aides in the provision of customer care
- Monitor the coordination of customer care services
- Conduct initial and ongoing customer assessments and communicate findings to all members of the interdisciplinary team
- Review and develops the Customer Service Plan
- Identify the ongoing need of all customers in order to maintain their health and wellness
- Discuss all service level recommendations with Clinical Manger
- Is on call for questions regarding the medical questions of the customer
- Provide nutritional counseling on special clients
- Provide emergency first aid to customer when needed
- Monitor all infections and/or accidents/incidents of customer. Monitor compliance of all infection control policies
- Maintain inventory of medical supplies
- Consults with physician when necessary to ensure and maintain safe, efficient and quality customer care
- Keep and maintain records of case assignments and case management
- Maintain clinical files and ensure proper documentation
- Review all customer or team member incident reports and develop a plan of correction to increase safety for both customers and team members
- Review the accuracy of all physician order quarterly and as needed
- Monitor customer’s medication regime
- Provide consultation and technical assistance to team members as needed
- Maintain customer confidentiality and coaches all Team Members on importance
- Coordinate customer service needs with other team members
- Participate in any requested Case Manager conferences
- Assist in counseling customers and family members in available services
- Attend meetings as requested and required
- Advise Clinical Manger of any need for a team member training or other actions when appropriate
- Develop a working knowledge of all Home care policies and procedures
- Participate in the Quarterly Medical Record Review process

Qualifications:
- Management skills and supervisory skills of paraprofessionals
- Has an understanding of the complex issues surrounding geriatric care
- Has genuine concern and compassion for seniors and their families
- Able to make responsible choices, decisions, make decisions and problem solve
- Be flexible with the ability to prioritize, good organization and time management skills RN, BSN Preferred but not required
- 3-5 years in acute care, assisted living and/or home health preferred but not required
- Possess and maintains a current and valid license in the State under the Professional Registered Nurse Law
- Case Management experience preferred
