Case Manager (RN) - FT Days

Accountable for the execution, adaptation, and outcome of care management as proscribed in the initial discharge plan with regard to resources, appropriateness of care, and adherence to a designated Goal Length of Stay (GLOS) for each patient. Must be proficient at coordination of care, implementation of care maps, and application of continued stay criteria for concurrent review. Works with physicians, nurses, ancillary staff, and social services at the point of care to facilitate multi-disciplinary decision-making that is consistent with the goals and objectives of the plan of care and the wishes of both the patient and family. Must also understand payer issues with regard to patient management and resulting implications of clinical decisions, and anticipate opportunities to reduce expense and capture revenue appropriately from admission through discharge.

-Current GA RN license

-3 years of RN Case mgmt experience required; 1 year Denial mgmt experience preferred

-Understanding of case management, discharge planning, and utilization review (UR) role related to the continuum of care needs of the patient population. -Must demonstrate strong negotiation, listening, assessment, planning, coordination of care and problem solving skills.
