POSITION SUMMARY :

The Registered Nurse in the Home Health setting coordinates with the interdisciplinary team to provide care to the client and his/her caregiver(s) in accordance with the plan of care, utilizing the Sutter Difference Standards and Standards of Excellence.

Principal Accountabilities

Community - Promotes a positive relationship between the community and the agency, and Sutter Health as a

whole, by acting as a liaison between the hospital, physician and client/family in accordance with the Sutter

Difference and Standards of Excellence. This also includes teaching clients and their caregivers how to utilize

healthcare resources within the community.

Finance - Utilizes resources to deliver quality care in a safe, efficient, and cost-effective manner.

Growth - Implements the client/caregiver plan of care to achieve the goals of care. This includes

assessments, nursing interventions and the teaching of the client-family unit to manage their care as

independently as possible.

People - Participates in the interdisciplinary team process in accordance to the Standards of Excellence, to

coordinate the care for the client/family. This includes ongoing coordination with all disciplines involved in the

clients'/caregivers' plan of care.

Quality - Provides optimal care to clients and their caregivers which is collaborative, evidence based and

meets regulatory, accrediting, and agency standards to achieve the goals of care.

Service - Cares for clients and their families according to the Sutter Difference standards and activities

ensuring the best client/caregiver experience. MINIMUM POSITION REQUIREMENTS

Education:

Current licensure as a California RN; Must have current American Heart Association BLS-HCPC recognition or Greater Sacramento Region - Healthcare CPR. Valid CA Drivers License, access to reliable auto, with proof of current auto liability insurance. Graduate of an accredited school of nursing; BS in nursing preferred.

Experience:

This position requires a minimum of one year experience in acute care or community setting. Home health experience is preferred. Senior Care: One year experience working with the elderly is required to meet federal/state PACE regulations.

Knowledge:

Must have demonstrated knowledge of the nursing process and community health care. Must have a working knowledge of applicable legal, regulatory, and accrediting standards. Demonstrates knowledge of the concepts of death and dying, utilizing the Hospice philosophy. Requires strong patient assessment skills as well as patient/caregiver education skills. Must possess basic computer literacy knowledge.

Skills:

Requires excellent organization, prioritization and communication skills. Requires effective English verbal and

written communication skills. Must work well independently, without significant supervision or direction.

Requires computer proficiency in Home Care Software (e.g. Horizon) and Microsoft Outlook. Must work well

independently, without significant supervision or direction. Skilled in the following clinical interventions: central

venous line management, pain management, wound management, venipuncture, blood glucose monitoring

and management of related medications and diet, coagulation testing and management of related

medications and diet, bladder catheter insertion and management.
