Heritage Senior Living is currently in search of a Wellness Director (RN) to work collaboratively with current and potential residents and their families on all aspects of wellness and care. Reporting to the Executive Director, the Wellness Director is responsible for all aspects of federal and state compliance and documentation, resident care, medication administration and training and supervisory management of all resident assistants/CNAs.



Essential Duties and Responsibilities




  • Complete a new resident assessment and move-in paperwork for admission (comprehensive medical provider report)

  • Complete an Individual Service Plan within thirty (30) days of admission and every six (6) months (or with change in condition)

  • Conduct daily check in with residents; identify ongoing needs in order to maintain health and wellness

  • Conduct wellness visits with resident families to ensure that all medical needs of each resident are met

  • Track physician appointments, contact physicians as needed acting as a liaison for all resident health services; document orders in resident chart

  • Ensure that staff review new resident needs prior to move-in, including any special dietary requirements

  • Review daily log each morning, report any incidents or concerns to supervisor

  • Monitor all infections and accidents/incidents; ensure compliance with all infection control policies

  • Monitor medication program including audits of records and procedures, staff training on proper procedures for medication passing

  • Monitor resident's medication schedule, maintain adequate supply of each medication (check in medication orders)

  • Communicate any medication and/or treatment changes to resident assistants/CNAs

  • On a quarterly basis, assess psychotropic medication needs

  • Process MAR sheets and document summary notes on a monthly basis for each resident

  • Conduct ISP meetings with appropriate teams; adjust if needed based on change in condition

  • Work with resident assistants/CNAs to regularly assess skills and make recommendations for training opportunities

  • Assist with OSHA compliance requirements for all employees

  • Develops working knowledge of all federal, state and company regulations, policies and procedures; ensure compliance

  • Conducts tours and takes inquiries as needed

  • Manage staffing schedule with Executive/Community Director



Education / Qualifications




  • Bachelor's degree in nursing; valid and current nursing license in State of Wisconsin in good standing

  • One (1) year of nursing experience working with seniors; experience within an Assisted Living, Memory Care or other long term care environment preferred

  • Managerial experience preferred

  • Must be willing to work a flexible schedule including nights, weekends, and holidays rotation

  • Excellent interpersonal skills - ability to connect with residents, families, and associates

  • Strong customer service orientation to older adults, exhibit a caring and compassionate attitude

  • Ability to make responsible choices and decisions that are in the best interest of the resident's

  • Ability to work collaboratively with all coworkers

  • Excellent written and verbal communication skills; must be proficient in Microsoft; Yardi software experience a plus

  • Ability to problem solve and communicate complex issues

  • Ability to work flexible hours and be available to respond during on-call rotation

  • Must submit to all applicable pre-employment and annual tests per state and federal regulations
