Home Instead Senior Care, one of the most trusted name in home health care, is currently seeking a Per Diem RN to manage and support of Home Health Aides (HHAs).
In addition to providing training and staff orientation, you will also be required to perform, in-home assessments and start of service evaluations.
Responsibilities include (but not limited to):
- Develop and implement treatment plans in conjunction with patient's authorized medical practitioners
- Reassess patients ongoing needs for services and discuss /document any changes in the plan of treatment
- Plan, facilitate, and document patients discharge from agency
- Perform in-home assessments and subsequent re-assessments of newly referred patients
Requirements:
- NYS licensed and currently registered as a Registered professional Nurse
- Working knowledge of NYSDOH rules and regulations is strongly desired
- Strong, problem solving, organizational, written, communication, and interpersonal skills
- Must be able to provide verifiable personal and professional references
- Must have regular access to the internet/email, and a mobile phone
