About this job
The Practice Coach is responsible for facilitating physician and practice processes to ensure patient data is completed in the population management software for chronic diseases.
Assists Quality Performance Specialists in providing training to practices in LEAN processes, disease registry training and tracking for accurate processes based on meeting Patient Centered Medical Home and Neighbor guidelines.
- Individual will be working hand in hand with doctors offices to update information in the database and gather files.
- Individual will be working with medical records and this is a highly confidential role due to the patient information.
Requirements
2 year degree in healthcare related field
2 years minimun experience as a Medical Assistant
