The First Home Care team has developed an integrated continuum of care which includes Therapeutic Foster Care, School-Based Programming, Alternative Day School and Outpatient Counseling. First Home Care Corporation is comprised of First Home Care (FHC) and Oyster Point Academy. In addition, FHC has a long-standing management agreement with Foundations for Home and Community-a Washington D.C. and Maryland based non-profit corporation.

First Home Care currently has an excellent career opportunity for an experienced PRN-Day Treatment Specialist to work in the Portsmouth, VA area.

The candidate selected will be responsible for the following:

Provides at least 2 separate therapeutic interventions as well as 1 hour of direct face-to-face time to each client per day during Individual and/or Group Activity Sessions as outlined on their Individualized Service Plan (ISP). Keeps Supervisor up-to-date on clinical issues including but not limited to changes in legal status, guardianship, educational placement, current behaviors, progress towards treatment plan, and other relevant information. Notifies Supervisor of pending discharges in advance of the discharge date. Develops partnerships and open lines of communication with outside agencies with whom you have a Release of Information that are involved in the treatment of the client. Documents contacts as required. Acts as advocate for assigned clients and their families. Safely transports clients as needed. Provides family contact/counseling either face-to-face or over the telephone for each client at least weekly and as needed throughout the week. Documents contact and submits documentation on a weekly basis. Provides original documentation of services rendered to the client in the form of a BIRP note within timeline set by Supervisor that contains no more than three (3) grammatical/spelling errors. Documents all incidents in accordance with policies and procedures, Incident Reports (where applicable) and Human Rights Reports within 24 hours to Supervisor and to Safety Designee. Utilizes appropriate crisis intervention/de-escalation strategies as outlined in NCI training. Develops, implements and updates client focused ISP's that are measurable, achievable, include required goals and objectives, and are unique to that specific client. ISP's are submitted in accordance to Medicaid timelines, includes all necessary signatures, detailed explanations of missing signatures including attempts to involve client's guardian in the ISP planning and review meetings, and contain no more than 3 grammatical/spelling/formatting errors. Documents appropriate narratives/coding on previous ISP to track client's progress of treatment goals. Completes suicidal risk assessments and safety plans with clients displaying suicidal gestures or ideation and staffs with Supervisor immediately. Revises client's ISP accordingly. Completes discharge packet that includes: D/C Letter, D/C Summary, Freedom of Choice form, and D/C Outcome in accordance to required timelines. Actively attempts to involve client and guardian in the discharge process and transition planning. Prepares monthly or quarterly reports for each client and submits to the Supervisor within 2 business days of the end of the reporting period with no more than three (3) grammatical/spelling errors. Ensures that monthly or quarterly reports are clinically sound, specifically address all ISP goals, and include updated information such as changes to diagnosis, medication, current behaviors, educational issues, and environmental issues. Monthly or quarterly reports are unique to each client for that specific reporting period.

The following are the job requirements for the Day Treatment Specialist position:

Bachelor's degree in Human Services field. Human Services field to include Social Work, Gerontology, Psychology, Psychiatric Rehabilitation, special Education, Sociology, Counseling, Vocational Rehabilitation, Human Services Counseling and Education.

One or more years of clinical experience. Must possess knowledge of DSM.IV diagnoses and have the ability to implement appropriate clinical intervention based on the following theories: Attachment, Cognitive Behavioral, Family Systems and Trauma. Must have valid driver's license. Must be a QMHP-C.
