Job Summary:

The program coordinator assists and collaborates in the functioning of the Primary Stroke Center components, which includes Hospital & Administrative Support, Acute Stroke Team, Written Care Protocols, Emergency Medical Services, Emergency Department, Stroke Units, Neurological Services, Neuroimaging, Laboratory Services, Outcome/Quality Improvement, and Educational Programs.

Clinical expert serving as a role model, educator and resource person to facilitate safe passage for patients with CVA/TIA. This position will ensure professional development of clinical staff and improve community awareness regarding stroke. In collaboration with the SERMC Stroke Committee will provide leadership and facilitation to promote improved CVA/TIA patient outcomes.

Essential Duties:

Coordinate the development and certification of the Primary Stroke Center under the leadership of Administration and the Stroke Medical Director. Once certified, then follow appropriate guidelines/recommendations to maintain certification.

Identify and follow patients with stroke during hospitalization for the purpose of applying evidence-based practice to improve patient outcomes, both physiological and financial.

Mentor staff in using/fostering the use of evidence-based tools to improve the care of a stroke patient (stroke order sets, nursing care resource guide, patient educational tools). Facilitate neurological assessment skills for caregivers across the spectrum of stroke care. Maintain hospital-wide communication regarding Stroke Program.

Develop and facilitate ongoing community and professional education programs. Assist with inpatient stroke education.

Participate actively in the continued development of improved care practices for patients, including practice guidelines and protocols through the use of evidence-based practice

Demonstrate motivation for continued growth as an educator, and by active review of current literature, professional networking, and attendance at relevant seminars/conferences. Communicate educational offerings to Stroke Team and keep track of the team's stroke educational hours.

Consults with OT/PT/Dieticians/Case Management/Social Work and other members of the health care team, to continue to identify new strategies for excellence.

Review stroke related data, disseminate with appropriate team members, and establish performance improvement plan to make changes.

Promote SERMC Primary Stroke Center in collaboration of the Marketing Department by means of appropriate printed patient educational materials, videos, billboards, etc.

Chairperson of SERMC Stroke Steering Committee.

Hours: Variable based on meetings and events requirements.

Education:

1. Bachelor of Science in Nursing (BSN) required.

Licensure/Certification:

1. Valid Nebraska RN license is required.

2. Valid Basic Life Support (BLS) certification required.

3. Hospital provided stroke course completed and annual requirements to maintain certification required.

Experience/Pre-Requisites:

1. A minimum of 2 years of healthcare experience required and/or demonstrated knowledge of healthcare operations.

2. Preference for demonstrated experience in developing and implementing customer service initiatives. Must possess strong business acumen.

3. Excellent oral and written communication skills; clearly and succinctly articulates thoughts and ideas, builds consensus and is a persuasive presenter.

4. Demonstrates a positive approach to problem-solving and issue resolution.

5. Excellent organization skills

6. Ability to work independently and with a wide variety of people.

7. Ability and desire to work in a team environment, handle multiple demands and to respond rapidly to changing priorities.

8. Computer and software skills including word processing and database management are preferred.

Professionalism/Growth and Development:

1. Active professional membership in nursing organization is required.

2. Knows, interprets, and practices safety, security and infection control policies and procedures of the hospital.

3. Practices proficient use of supplies and equipment with awareness for cost containment for patient and hospital.
