Providence is calling a full time Patient Care Coordinator / Community Liaison Nurse to Providence Home Health and TrinityCare Hospice in Torrance, CA.
This position will be based in Torrance and will cover the greater South Bay area, UCLA and Cedars-Sinai. This is eligible for a $5000 sign-on bonus.
In this position you will:
Act as the Company's representative, promoting services, identifying new opportunities, processing referrals, communicating with patients, family and physicians, providing Intake with the necessary information to service the patient.
Communicate with physician hospital staff, client, caregiver, and agency staff in order to provide continuity of care in the transition from the hospital or SNF to home.
Serve as a resource and educator to physicians, hospital staff, clients, caregivers, and community at large regarding home health.
Required qualifications for this position include:
Current California Registered Nurse ( RN ) license
Minimum 1-year of acute care, home care or hospice experience within the past three years
Personal transportation, with current auto registration
Computer skills/knowledge, including experience with electronic medical charting
Preferred qualifications for this position include:
Marketing and/or community outreach experience
Recent experience in hospice strongly preferred
Discharge Planning experience a plus

Caring for people in Los Angeles and Orange Counties with professional teams. Providence TrinityCare Hospice is Medicare Certified and CHAP (Community Health Accreditation Program) accredited. We also provide the only specialized pediatric hospice service in Los Angeles County.
As the only pediatric hospice in Los Angeles County, Providence TrinityKids Care understands the impact a seriously ill child can have on the life of a family. A specialized division of TrinityCare, TrinityKids Care helps children with life-limiting illnesses leave the hospital in order to spend as much time as possible at home with their families.
