We are currently seeking a Social Worker to join our Disease Management (DM) team. The Social worker will provide telephonic direction for routine aspects of non-medical problems of patients and their families. Also acts as a resource for information about and referral to other community based services. They function as a member of the departmental team which includes educating the patient, family and members of the healthcare team regarding benefits, community resources, referrals for counseling and other pertinent information.

This is an office position at 2222 W. Dunlap Ave. Phoenix, AZ   

Primary Responsibilities:    

Collaborates with the DM staff, beneficiaries, families, caregivers, and community agencies to facilitate the appropriate and efficient delivery of health care services utilizing beneficiary benefits structures and community resources
Assess available services to meet the social and emotional needs that affect a patient’s medical condition, treatment, recovery and transition to a lower level of care
Assure that patients and families are knowledgeable of available community resources and how to access them
Serves as a resource to clinical staff in dealing with complex psychosocial issues and accessing community resources
Research and investigate other community and government based programs and organizations to improve access to services
Act as a liaison to social services and government agencies

Required Qualifications:   

Current, valid, unrestricted,  LCSW, Licensed Clinical Social Worker or Licensed Masters Social Worker
3 years of experience as a Licensed Social Worker
Skilled at researching community resources for out of state participants
Self-motivated and proactive approach to work load
Intermediate to Advanced computer skills-Microsoft Word, Outlook and Internet Explorer, with the ability to navigate a Windows environment
Good interpersonal skills, effective verbal and written communication skill
Excellent clinical skills and judgment
United States Citizenship
Ability to obtain favorable adjudication following submission of Department of Defense form SF86 NAC form SF86
Preferred Qualifications:   

Knowledge of health care and insurance industry including Medicaid and/or Medicare, regulatory and compliance
Previous disease management experience
