**WEEKEND ON-CALL CAREGIVER**
Responsibilities:
*INCLUDING BUT NOT LIMITED TO:
*Companionship
*Assistance with Ambulation and Safety Assist
*Meal Preparation
*Medication Reminding
*Light Housekeeping
*Providing Physical & Emotional Support
*Personal Care Assistance with tasks such as bathing, grooming, toileting, incontinence care
Requirements:
*Caregiver MUST be available for ALL schedules during the period of Friday 4:30pm through Monday 8:00am and ALL holidays
*Caregiver MUST agree to travel entire service area
*Caregiver MUST be able to service clients who smoke and/or have pets
*Caregiver MUST be able to provide PERSONAL CARE to BOTH MALE AND FEMALE
*Caregiver MUST be responsive to Administrative On-Call Supervisor (30 minutes or less)
*Failure to fill any schedule request during ON-CALL period (Friday 4:30pm through Monday 8:00am & holidays) will result in forfeiture of ON-CALL compensation and termination
*Caregiver will be compensated $50 per weekend or holiday ON-CALL periods whether used by Administrative On-Call Supervisor or not
*In addition to ON-CALL pay, any schedules filled by the On-CALL Caregiver will be compensated HOURLY, LIVE-IN, OR OVERNIGHT pay (depending on what schedules are filled) per your Employee Hand Book
*MUST AGREE TO SIGN A "LIVE-IN" AGREEMENT & PROVIDE TIMESHEETS per AGREEMENT
*Caregiver MUST be available to arrive in a respectable amount of time to the client's home once requested to do so (within 1 hour or less)
*Caregiver will follow ALL policies and procedures in the Employee Handbook
*Caregiver MUST sign On-Call Caregiver Agreement
*Complete a Criminal Background Check, DMV Report, Pre-Employment Drug Screening
*Provide 3 PROFESSIONAL References & 3 PERSONAL References
*Possess a VALID ILLINOIS DRIVERS LICENSE
*Possess VALID AUTOMOBILE INSURANCE
*ABILITY TO TREAT AND CARE FOR CLIENTS and their property with DIGNITY & RESPECT
*ABILITY to COMMUNICATE with Clients in a FRIENDLY and CONGENIAL MANNER
