Primary Responsibilities:

Verify Program Eligibility
Coordinate outreach to schedule and confirm home visits with NP’s
Track all inbound and outbound calls and NP visits
Documents all activity in electronic health record
Update and upload patient care data
Support NPs in the field to verify directions, reschedule visits, make referrals
Identify and correct inconsistencies in documentation demographics/notations
Fax NP encounter assessments to PCPs
Communicates and collaborates with other members of the team (NP, NCM, manager) to coordinate care and services
Process referrals for durable medical equipment and referrals to specialists as directed
Track hospital admissions and discharges
Submit lab orders
Document case information completely, accurately, and in a timely manner.
Participate quality monitoring process
Demonstrate a high level of customer service  behavior at all times
Expedite emergency calls as needed
Attend regular staff meetings as scheduled
Coordinate inbound/outbound calls  – patient calls, appointments, DME, referral updates
Maintain confidential health information according to state and federal regulations including HIPAA. 
Assist with gathering and organizing of appropriate medical records (e.g. history and physical, lab work, test results).
File and fax
Provide ongoing support and education to team members.
Assist to onboard new hires
Provide subject matter expertise on PCC processes, procedures and workflows
Prepare materials, power points, attendance rosters, to support staff meeting.
Coordinates appropriate referrals to case management staff 
Demonstrates sensitivity to issues and show proactive behavior in addressing customer needs.
Conducts urgency screens of new members and triages as needed
Monitors EMR for note completion and assigns patients to appropriate provider and program
Develops program reports for internal and external customer
Prepare materials for meetings. 
Coordinate supplies for providers in the field, ordering, distributing etc.
Gather and organize information for leadership to distribute to team
Perform data collection, data entry and quality monitoring activities, as applicable
Performs other duties as assigned by supervisor.

Requirements

Required Qualifications:

High School Diploma/GED
Healthcare experience
2+ years of telephonic Customer Service experience
1+ year of a healthcare background with medical terminology and familiarity of clinical issues
1+ year of experience working with Microsoft Word and Excel

Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area

Preferred Qualifications:

Associate’s Degree (or higher)
Experience working with chronically ill population
Soft Skills:

Strong Organizational skills
Strong Verbal and written Communication skills
