REM Wisconsin is seeking a highly motivated, self-starter to serve as Program Coordinator to manage the administrative duties and provide direct care for day and evening shifts at a 4-bed Adult Family Home in River Falls. The Program Coordinator ensures all clients are properly managed and cared for, including implementing programs and services for the clients, coordinating activities, and scheduling appointments.

Description

Key Responsibilities:

· Manages the day to day activities, budgets, and schedules for the home and designated clients supported. Manages and schedules staff.

· Supports clients as a Direct Support Professional the majority of hours worked.

· Maintains census and maximizes utilization and occupancy percentages within programs.

· Implements program plans and submits incident reports as appropriate for individuals receiving services (clients).

· Implements safety plans for clients supported and coordinates scheduling. Ensures clients are receiving appropriate health and medication management. Assists in the management of client funds.

· Ensures home is clean, neat, and safe; and works with and/or contacts maintenance as appropriate.

· Performs other related duties and activities as required.

Qualifications

Requirements:

  • High school diploma or GED
  • Three years related work experience
  • Must be 18 years of age
  • Valid driver’s license in good standing
  • Car registration and vehicle insurance if providing transportation for individuals receiving services
  • Successful clearance of local and/or state background checks
  • Supervisory experience preferred
