Description

CareMeridian is currently seeking an immediate need for a Director of Nursing (DON) to improve and sustain superior performance of the facility as evidenced by quality outcomes . A strong emphasis is placed on CareMeridian’s mission and values.

We are looking for a strong yet compassionate person, who will use their skills and experience in our home like facility located in Fairfax, Ca in Marin County.

This person should be able to demonstrate a desire for strategic patient outcomes, and the ability to manage personnel and staff while demonstrating key qualities of leadership.

Essential Duties and Responsibilities:

  • Plan and organize the daily functions of the Nursing department.
  • Participate in the development and updating of the interdisciplinary plan of care.
  • Complete pre-admission resident evaluations at their current place of care.
  • Ensure that all Safety Policies and Procedures are followed at all times, including timely completion and submission of all incident and accident reports.
  • Ensure compliance with all regulatory guidelines, licensing requirements.
  • Reviews, manages and supervises case management to include:
    • Admission process and resident business files
    • Resident funding
    • Additional funding resources
    • Discharge planning
    • Interdisciplinary team conferences
    • Maintain staffing levels within regulations and budgetary guidelines with assistance of corporate assistance and with the support of the Regional Director for Northern CA operations.

Qualifications

Requirements:

  • Current and active Registered Nursing (RN) license issued by this state.
  • Preferably three (3) years experience working with medically complex individuals and in the nursing profession as an RN.
  • Preferably three (3) years experience in a leadership/management role.
  • Able to effectively communicate (verbally and in writing) with physicians, staff, residents, and their families and/or authorized representative, and other members of the community.
  • Experience with spinal chord, brain injury and high acuity of care preferred but not required.
