Job Summary This job opening is for a permanent LPN position in the Post-Transplant Clinic in Cardiology with Internal Medicine.

Responsibilities* This position's responsibilities include:

Assist in the delivery of direct patient care of patients followed by the post-transplant service. Provide patient instruction, education regarding the heart transplant, service guidelines, and communicating routine patient laboratory results. Review and send (fill) in prescription requests and/or forward to AP providers or MD as appropriate. Obtain prior authorizations for patient's immuno-suppressive medications and other meds associated with post heart transplant. Support Post Heart Transplant Coordinators with no change lab letters and arranging next lab dates. Provide routine patient instruction and education for new patients referred to the post-transplant service. Review program information, patient responsibilities, and routine patient instructions for the telemanagement clinic. Support APP's/MD's with FMLA forms, Disability forms, Cardiac clearance forms, and other medical documents. Review spread sheets for tracking of post heart transplant related cardiac testing. Complete routine patient phone calls, communicate normal laboratory results and follow-up laboratory appointment information. Assist with triaging patient calls as delegated by nursing staff.

Required Qualifications* Graduation from an approved school for practical nursing is required. Current licensure as LPN in State of Michigan is required. Recent experience working in a clinical setting with patients. Ability to prioritize and problem solve is required. Demonstrated ability to work effectively with a multidisciplinary team is required. Excellent customer service skills are required. Good computer skills. Always strives to be part of the solution rather than part of the problem by offering constructive, creative ideas.

Desired Qualifications* Previous experience in Telemanagement clinic. Experience with obtaining prior authorizations from insurance companies. Previous experience in ambulatory care. Previous experience in a transplant program. Knowledge of University policies and procedures. Knowledge of MiChart, Microsoft Office & Outlook, OTIS Demonstrated accountability/dependability as evidenced, for example, by a stellar attendance record; by completion of mandatories and unit based competencies within an established time frame; by meeting deadlines and completing required work.

NOTE: This position is directly responsible to the Nurse Manager and Supervisor with functional reporting to the nursing staff in the post-transplant service.

Work Schedule Hours: 40 hours, 5 days per week, Monday - Friday

Shift: 8:00 am - 5:00 pm
