will act in a liaison role with Medicaid members to ensure appropriate
care is accessed as well as to provide home and social assessments and
member education.Primary Responsibilities:Create a positive experience and relationship with the memberKeep the member out of the hospital by supporting regular visits to their primary physicianKeep member actively engaged with their primary physicianSupport the member to ensure pick-up of their RxProactively engage the member to manage their careProvide member educationSupport transitions of careHelp to keep members compliant with their care plansPartner with care team (community, providers, internal staff)Knowledge and continued learning of community cultures and valuesMay conduct member assessments if needed

Requirements

Required Qualifications:High School Diploma or GEDAvailable to work 8 hours a day, 40 hours a week between 8:00 am - 7:00 pm (may change based on business needs)Must have resided within the local community for 2+ yearsKnowledge of culture and values within the local communityFamiliarity with the resources available within the communityField Based experienceAccess to reliable transportation that will enable you to travel to clients and/or patient sites within a designated area.Must live in Milwaukee County, WIPreferred Qualifications:Bachelor's Degree in Social Work and/or Health Care AdministrationLPN (Licensed Practical Nurse) licensure, CNA/HHA, or Medical AssistantBilingualCommunity Health Worker (CHW) AccreditationExperience working in Managed CareKnowledge of WI Medicaid & Medicare populationCareers with Optum. Here's
