Recent retirees welcome! Home Instead Senior Care is looking for a part-time Licensed Practical Nurse or Registered Nurse to complete in-home care assessments in the Northern Neck and Middle Peninsula. Primary responsibilities include, but are not limited to:
• Reflect the Company mission and core values
• Demonstrate open and effective communication with owner, colleagues, supervisor and CAREGivers
• Ensure compliance with the Virginia licensure requirements and participate in audits
• Complete a Client Plan of Care within four weeks of signing up a new client
• Prepare and execute Department of Veterans Affairs client assessments
• Consult with staff coordinator(s) to assign qualified CAREGiver to individual clients
• Reevaluate client’s needs and check for quality assurance every 90 days during an in-home personal care assessment visit
• Report changes in client’s medical or mental condition to agency employees involved in the client’s care or other appropriate medical personnel
• Enter all appropriate information in Proprietary Software
• Conduct Personal Care Training for all newly hired CAREGivers
• Assess and document performance of CAREGivers in client’s homes
• Evaluate and update assessment and training documents as necessary
• Prepare, execute and document 12 hours of in-service hours for all CAREGivers
• Prepare, conduct and fill out appropriate paperwork for CAREGiver annual evaluations
• Review and approve client care logs
This position is considered part-time and will most likely involve up to 24 weekly work hours.
