Clinical Nurse Liaison - Gainesville, FL - Full-time / Part-time

Under moderate supervision, provides clinical education and marketing services to hospital and physician referral sources to ensure awareness of AxelaCare and AxelaCare family of companies programs and services.  Coordinates home infusion nursing services with appropriate home infusion candidates.   
 
***This position will cover UF Health Shands Hospital in Gainesvile, FL***
 
Schedule is Monday - Friday from 8:30am - 5:30pm, with flexibility needed
  
Primary Responsibilities: 


Understands all therapies provided by company and is able to communicate methods of administration to patients and caregivers
Maintains and grows core therapy admits through customer relationships and provides responsive customer service with assigned accounts.  Evaluates current and future clinical and service needs for existing and new accounts
Effectively communicates with branch and exhibits professional behavior. Provides excellent customer service to external and internal customers
Demonstrates thorough knowledge of core therapy referrals, qualifying criteria both by payor source and branch
Collaborates with case managers, payers, referral sources, home health agencies and AxelaCare staff to facilitate patient discharge and coordination of care; Conducts in - services and continuing education programs for referral sources and / or their support staff; Introduces new services, therapies, drug protocols and products to referral sources; Recognizes opportunities for expanding the business relationships with existing accounts and involves the Regional Sales Manager at the appropriate time to present and negotiate AxelaCare’s other programs
Provides clinical expertise in support of the sales effort by utilizing clinical skills to assist referral sources with identification of appropriate home infusion candidates, providing assessment of patients’ ability to provide self-care, providing patient teaching regarding therapy, supplies, and safety and / or by connecting patient to a pump in order to facilitate discharge from a facility
Appropriately documents patient care activity, coordination of home infusion nursing services provided and all communication with patients, physician and referral source; Documentation is legible, accurate, complete, and complies with company policy and procedure for electronic and paper patient clinical record entries; Responsible for obtaining and sending all required documentation from the clinical setting to the branch office in a timely manner
Communicates / coordinates / collaborates discharge planning activities for new patient referrals with branch personnel (admissions, clinical, operations, reimbursement and referral sources)
Answers incoming telephone calls and triages to appropriate person as indicated 
Participates in staff meetings with nursing and pharmacy
Participates in departmental QA activities as requested
Participates in cross training of the various clinical functions in order to function in multiple roles as required
Participates in “on - call” rotation responsibilities as required
Attends designated departmental and branch meetings as scheduled
Adheres to the policies and procedures of the company
Completes paperwork and reports and accurately submits to supervisor according to policy requirements
Performs other tasks or duties as requested
 

Required Qualifications: 


Associates or Bachelor’s degree from an accredited college or school of nursing
Minimum of two years of IV related experience 
Current licensure as a Registered Nurse in the state of Florida. Additional licensure may be required in multi-state service areas
Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area
Effective communication skills with patients, family members, other staff members and outside organizations or persons
Basic typing / data entry skills
Possess the initiative to plan for and complete daily activities with minimal directions
Dependable in attendance and job performance
Ability to meet attendance, overtime, on - call, and other reliability requirements of the job
