Registered Nurse - Full-time / Part-time

The primary duties of this position are to coordinate patient care with the interdisciplinary team including physician, patient/family, and home health aide; and administer skilled nursing care for EEOICPA clients in their home or place of residence and assume the responsibility for coordination of care.
*Supervises and provides directions to the HHA to ensure quality and continuity of patient care
*Reviews and coordinates the completion of HHA care notes
*Assures continuity of quality patient care delivered with required documentation
*Monitors assigned cases to ensure compliance with requirements of the Department of Labor
*Performs on-call responsibilities and provides on-call services to patients/families, as assigned
*Provides effective communication to patient/family, other team members and other health care professionals
*Adherence to HIPAA and compliance guidelines
