Join us and help people live healthier lives while doing your life's best work.(sm)  The Senior Case Manager reports to the Clinical Service Manager in support of Nurse Practitioners in the field. This position will mainly cover the Middlesex and Hartford County territory and is approximately 75% traveling in the field and 25% home-based. Primary Responsibilities:

Nursing home chart reviews for ICD- 9 coding and medication reconciliation
Diagnosis reviews for coding accuracy
Transcribe on-call logs into electronic medical record
Provide in-services to partnering nursing home staff for development and new home openings
Assist NP's with health assessments
Obtain vaccination reports from nursing homes and enter information into patient electronic medical record
Monitor use of level 2 bed support surfaces to meet Medicare criteria.
Must travel to nursing home facilities daily throughout primarily southern CT New Haven to Greenwich
Driver's license, reliable means of transportation and appropriate insurance  coverage
Excellent communication and interpersonal skills

Requirements

Required Qualifications:

Registered Nurse (RN) with an active and unrestricted license in CT
5+ years clinical experience as a RN in a hospital, acute care, home health / hospice, or nursing home
Computer skills (Microsoft Office) and competency with electronic medical recordsPreferred Qualifications:

Home care or nursing home experience
BSN  
ICD-9 coding experience / knowledge
Case Management experience
Medicaid, Medicare, Managed Care experience
