Start doing your life's best work.The LVN Field Case Manager for the STAR Kids program will manage the care of members in their homes or other community settings by completing in-person health assessments and coordinating patient care.  This candidate may identify, coordinate, or provide appropriate levels of care under the direct supervision of an RN.  Work is generally self-directed and not prescribed; this person will complete face-to-face member assessments, so it will be important to function in a less structure work environment. Primary Responsibilities:Engaging members / families in person (in their homes or in a facility) to coordinate services, community resources, and treatment needsTravel in assigned territory as needed to complete member assessmentsDelivering a holistic approach to coordinated care based on the member's needs using a person centered philosophyDocumentation in an electronic fileFunctioning as an advocate for the membersBuilding relationships with the member and their family to assist with coordination of health choicesEnsuring documentation is completed in a timely mannerCoordination with other state agencies, community resources, and providers

Requirements

Required Qualifications:High School Diploma or GEDCurrent and unrestricted LVN License in the State of TX1+ years of experience in Service Coordination or Case Management High Speed Internet access in homeStrong computer proficiency (ability to create, copy, edit, save and send documents utilizing Microsoft Word, Excel, Outlook and Internet)Experience with pediatric clientsAbility to adhere to state mandated deadlinesAccess to reliable transportation that will enable you to travel to client and/or patient sites within a designated area Preferred Qualifications:Home Health experienceExperience with Medicare and/or MedicaidManaged care experienceBilingual in English and SpanishCareers at UnitedHealthcare Community & State. Challenge brings out the best in us.
