The Hospice Field Social Worker is a member if the interdisciplinary team whose primary function is to provide diagnostic assessments of the psychosocial and economic needs of the patient/family unit, define social service goals for alleviating these needs and help provide both the counseling and casework needed to meet these goals.
 
*Standard hours: 8:00am - 5:00pm, 1 night a week on call 5:00pm - 8:00am and rotating weekend call, Friday 5:00pm - 8:00am Monday*
 
*This is a full time position that requires travel throughout our hospice service area*
 
Primary Responsibilities:

Visiting patients to assess care and provide pain / symptom control support for grief, anxiety and feelings at end of life
Managing a case load of Hospice patients / families
Participating in care conferences and working in interdisciplinary teams
Supporting the patients’ families with funeral arrangements, psychosocial needs, community resources, etc.
Attending weekly interdisciplinary team meetings
Maintaining hospice patients’ charts and other documentation
Participating in a required rotating on call schedule that includes nights, weekends and holidays
Must travel to nursing home facilities daily throughout the St. Louis and Washington (Franklin county)



 
 
 
 

Requirements

Required Qualifications:

Master's Degree-level clinician in Social Work
Licensure in the following: LMSW and or LCSW
2 years of Social Work experience
Computer proficiency with MS Office products including Word and Outlook
Preferred Qualifications:

Hospice experience
Experience working with patient and families dealing with life-threatening illness and death
Bilingual (English, Spanish)
Experience working with community resources
Home health experience
Long term care experience
