And we do it all with every action focused on our shared values of Integrity, Compassion, Relationships, Innovation, and Performance.As a Community Health Worker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education.Primary Responsibilities:Create a positive experience and relationship with the memberKeep the member out of the hospital by supporting regular visits to their primary physicianKeep member actively engaged with their primary physicianSupport the member to ensure pick-up of their RxProactively engage the member to manage their careProvide member educationSupport transitions of careHelp to keep members compliant with their care plansPartner with care team (community, providers, internal staff)Knowledge and continued learning of community cultures and valuesMay conduct HRA assessments if neededRiverton

Requirements

Required Qualifications:High School Diploma/GED Must have resided within the community for 2+ yearsKnowledge of culture and values of communityFamiliarity with the resources available in the communityField based experienceComfortable traveling up
to 80% of the time (up to 80 mile radius)           Comfortable working an 8 hour shift between hours of 8:00 am - 7:00 pm Monday-Friday (may change based on business needs, must be flexible) Preferred Qualifications:Bachelor’s Degree (or higher) in social work and/or health care administrationLicensed practical nurse (LPN), Certified Nursing Assistant/Home Health Aide, Medical AssistantBilingual fluency in Spanish and EnglishCHW accreditationExperience working in managed careKnowledge of Wyoming Medicaid populationCareers with Optum. Here's the idea.
