Healthcare Social Worker - La Palma, CA - Full-time / Part-time

AppleCare Medical Management Group was incorporated in 2003.  Over the last 17 years, the multispecialty medical group of independent practices has expanded into new communities, contracted with more hospitals and health plans, and added over 50 new doctors - but the mission has remained the same. Our Social Worker’s assist the Manager of Social Services in the facilitation and coordination of community resources, in-home support services and other community-based services including Medi-cal waiver services for AppleCare managed care members, CalConnect Dual Demonstration Project, and ACO beneficiaries. 
 
Primary Responsibilities:


Assesses patient and family psychosocial needs and develops plan of care in concert with patient, physicians, nurses and other members of the departmental team

Links patients / family to appropriate community resources including but not limited to, information / referral to sources of financial assistance, transportation, support groups, and other community services

Documents patient / family status, diagnosis, treatment plan, goals, and interventions, evaluation results, observations and progress in medical records

Provides discharge planning based on meeting identified goals of the treatment plan anywhere
 

Required Qualifications:


MSW  

2+ years of acute care experience
Preferred Qualifications:


Experience in Hospice and Palliative Care

Knowledge of utilization coverage guidelines, coordination of benefits, and  coordination of care processes provided throughout the healthcare continuum.
