Spiritual Care Coordinator- per diem - Full-time / Part-time

Responsible to provide spiritual support to clients, families, and the hospice staff via phone contact, home visits and referrals to the client/family, local clergy. Assists with memorial services and activiites relating to spiritual and religious issues of client, family and interdisciplinary team. 4701 - Heartland Hospice - Serving South Minnesota Experience in client/family counseling. Demonstrated experience in working with clients and families coping with terminal illness and death. Excellent communication and interpersonal skills.
