MileStone Healthcare is currently seeking a Licensed Social Worker (BSW required) to provide Discharge Planning on a part-time basis at Sojourn at Seneca, a 24-bed Geriatric Psychiatric Hospital in Tiffin, Ohio.  Sojourn at Seneca provides a caring, supportive environment specifically designed to meet the mental health needs of the geriatric population in the Seneca and surrounding counties. Through a structured plan of individual counseling, group therapy, coordinated activities and family support, patients are given the opportunity to renew social, physical and mental skills.

This position will work 2-3 days per week providing Discharge Planning services.

Bachelor's degree in Social Work required; previous experience with Discharge Planning, preferably with senior/geriatric population.
 

Job Specific Details:

The ideal candidate will have prior experience in a geriatric, behavioral health or psychiatric medical setting with discharge planning a plus.  Must have a Bachelor’s degree and be a licensed Social Worker or eligible in the state of Ohio.  Work hours are flexible and can fit around your schedule!
