The position is a licensed practical nurse working with a Pharmacy Benefit Manager company in Orlando. This is a Full Time position under a (4) month contract with the opportunity to go permanent. Pay ranges from $22.00-$25.00 per hour, Monday-Friday 8 hour shifts, and a required open availability between 8am-7pm. The position is located near the Orlando Florida Mall.

This is a call-center/Specialty Pharmacy environment. This person would be responsible for handling a case load of patients divided among 2 other nurses. Provide clinical assessments and customer clinical management.

Perform Initial patient assessments on multiple disease states, home care coordination for injection training, coordination and documentation in the patient record, and clinical review of all information needed to complete the Schedule First Order Nurse function according to operational policy and procedures.

Provides clinical related information, evaluates patient clinical needs for specific therapies that treat chronic diseases, terminal illnesses, genetic disorders, bleeding disorders, and others within our Specialty Pharmacy. As Evidenced by: answering questions; researching information; completing requests, advising internal and external customer groups on drugs or drug therapies and disease states, nursing practice and company operations.

Ensure continuity of care by verifying, collecting, analyzing, organizing and reviewing patient data, medical histories, and evaluating, instructing, problem solving, according to specific plans of care for drug or drug therapies.

Drive patient compliance by reviewing patient profiles, answering patient medical inquiries, contacting appropriate medical professionals and referring patients, when needed, to the appropriate departments to assure adherence to plan of care.

Collaborate with all staff team members to ensure team effectiveness and productivity and offering suggestions for improvement.

Follows all internal processes and procedures to ensure activities are handled in accordance with departmental and company policies and procedures.

Perform Rx is looking for a LPN to join their small team of nurses. This is a call-center/Specialty Pharmacy environment. This person would be responsible for handling a case load of patients divided among 4 other nurses. They would be responsible for the initial assessment calls, on-going assessment calls regarding side effects. They would handle any triage calls from members and also council patients over the phone. They would record all the data in their systems, as well as use Excel, MS, and Outlook extensively.

They would also be responsible for talking to the providers(Nurses and Doctors) at the hospitals/other offices. They service members in SC, LA, FL, PA and MI. They handle Medicaid/Medicare patients, but also have commercial plans they service. One of their biggest plan is Prestige.

The disease states they would be working with are Hep C, Rheumatoid Arthritis, psoriasis, Cystic Fibrosis, MS, and Oncology.

They also handle patients on Synergist which are preemie babies. These cases are handled very carefully and have more steps then the other medications. Synergist requires you to follow up with the patients their first 5 injections. You would set up the orders of medication, but also track them. You are dealing with the parents of these babies, but also the nurses and doctors.
