Join our exceptional Optum Palliative Care team!  As a Nurse Practitioner on the Hospice and Palliative Care Team, you will provide palliative care services to patients in the hospital and facility base setting. This is an amazing opportunity that offers a flexible and autonomous schedule. As well as the satisfaction of providing quality end of life care to our aging population.
 
As a Nurse Practitioner with our program, you will collaborate with the Executive Director, Medical Director and Clinical Services Manager to ensure that palliative care consults and follow up visits are conducted in a timely manner.  
 
***This is a per diem (as needed), hospital and facility based position that will require travel throughout our service area in order to conduct palliative care consults and follow up visits. Weekend and weekday availability is required***
 
Primary Responsibilities:

Providing palliative care consults for patients in both the hospital and facility based setting
Visiting patients on an as needed basis to conduct follow up assessments and provide palliative care services
Conducting a patient / family evaluation and educating them on our palliative care services
Facilitating highly effective conversations regarding goals of care with patients, families, interdisciplinary team members, and other providers
Performing highly effective pain and symptom management treatment and care plan recommendation with Medical Director, families, and other providers to enhance patients’ quality of life
Participating with others to deliver plan for optimal coordination with primary physicians and other treating providers including the IPA and local care delivery system
Educating facility staff on our palliative care program
Conducting face to face Hospice specific visits as needed
Ongoing collaboration with the PCP and Medical Director regarding palliative care for the patients in our census
Collaborating with the nursing staff and the patients' families to ensure quality of care

Requirements

Required Qualifications:


Current, unrestricted NP license

Nurse Practitioner national certification in adult, family and / or gerontology

Active DEA

Prescriptive authority

CPR certification or obtain within 30 days of hire

Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area

2+ years of Hospice and / or palliative care experience

Intermediate level of proficiency in PC based word processing including Microsoft Word and Outlook
Preferred Qualifications:


Certification in hospice and palliative care (ACHPN)

Hands-on clinical experience as a nurse practitioner

Pain / symptom management experience

Hospital based experience

Critical care experience  

Long term care and / or nursing home based experience

Experience working with electronic medical records (EMR)
