We are seeking a knowledgeable, compassionate RN to handle patient visits on a PRN/as needed basis and will be required to travel throughout our service area to conduct the visits. As a Per Diem Hospice Nurse, you would work with a team of healthcare providers to ensure our Hospice patients (prognosis of 6 months or less) are receiving the right care. Additionally, you would provide emotional support to the patients' families. This position offers potential for growth, a friendly staff, and an opportunity to interact with patients and their families on a more personal level.
 
Please note: This position is on a Per Diem/PRN basis working as needed. The Per Diem RN will help provide visit coverage for vacations, call outs and PRN visits. This is a field based position and may include travel throughout our service area in order to conduct visits at our patient’s home or facility settings.

Primary Responsibilities:  

Conducting routine/follow up visits at home, skilled nursing, long term care and/or assisted living facilities
Coordinating, assessing, and implementing patients' care
Providing accurate and timely documentation of patient services to reflect plans of care
Conducting admissions, pronouncements and/or recertification visits
Interacting with other group members such as CNA's, Chaplains, social workers and management
Performing physical activities that require considerable use of your arms and legs and moving your whole body, such as climbing, lifting, balancing, walking, stooping, and handling of biohazard materials
Participating in an on call holiday rotation schedule

Requirements


Required Qualifications:


RN license in any state and must obtain a temporary or permanent RN license in GA upon hire
3 years of clinical experience
Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area
Flexible and adaptable with a willingness to work afternoons, evenings both during the week and on weekends
Ability to perform physical activities that require considerable use of your arms and legs and moving your whole body, such as climbing, lifting, balancing, walking, stooping, and handling of biohazard materials
Computer proficiency with MS Office products including Word and Outlook
Preferred Qualifications:

Hospice experience
Home care experience
Long term care experience
Case management experience
Proficiency with electronic medical records (EMR) software
