Work from home to provide as needed coverage for the after-hours on-call service
 
  Our per diem position offers the opportunity to work on our team of compassionate NPs to ensure continued care for our Senior members.   
 
Weeknight coverage for on call is 5PM -- 8AM Monday through Friday. 
 
Weekend coverage is divided into  two 12 hour time slots: 8 AM ---8PM and 8PM--8AM on Saturdays and Sundays.  
 
Rotating holidays are also included in the potential coverage needs.

In this role, you will provide telephonic on call medical management for patients in long term care facilities on weekends and holidays. You will collaborate with nursing home staff, PCPs and responsible parties on plans of care, and utilize electronic medical records for documentation.

Requirements

Required Qualifications:

Unrestricted NP licenses in the State of Florida required
Minimum two years of experience as an NP
DEA License or ability to obtain
Nurse Practitioner national certification as ANP, FNP, or GNP required or ability to obtain
A clinical background in adult, family or geriatric specialties
Intermediate level of proficiency in PC based word processing including Microsoft Word, Outlook and EMR
Must be able to work independently
