As a Cancer Support Program (CSP) RN Case Manager in our Atlanta office, you will review patient status over the phone while ensuring they are receiving the best care. We are currently hiring for shifts that end at 7PM EST Monday through Friday.

Primary Responsibilities:


Making outbound calls to assess oncology members' current health status
Identifying gaps or barriers in treatment plans
Providing patient education to assist with self-management
Interacting with Medical Directors on challenging cases
Coordinating care for members
Educating members on disease processes
Encouraging members to make healthy lifestyle changes 
Documenting and tracking findings
Making post-discharge calls to ensure that discharged member receive the necessary services and resources
Utilization management through evaluating and approving the medical necessity, appropriateness and efficiency of requested health care services, procedures and cancer treatments under the provisions of the applicable health benefits plan

Requirements

Required Qualifications:


A current, unrestricted RN license in the State of Georgia
3+ years experience in a hospital setting, acute care, direct care experience OR experience as a telephonic Case Manager for another insurance company
1+ years of clinical experience dealing with oncology patients  
Chemotherapy administration experience
Preferred Qualifications:

BSN
OCN certification
A background in managed care
Case or disease management experience
Experience / exposure with discharge planning
Certified Case Manager (CCM)
Bilingual (Spanish/English) skills
Computer proficiency, to include strong data entry skills and the ability to navigate a Windows environment
