Our providers focus on each unique patient to deliver the appropriate care and emotional support to our patients and their families. Our office staff supports our providers and our patients in real time on a daily basis to help to improve overall patient satisfaction.

Summary: Responsible for providing medical services and treatment to patients and families during home visits, principally in assisted and independent living facilities, serving as a clinical resource for staff and interdisciplinary team. Examines adult patients to ensure proper care and disease prevention, diagnosis, treatment, and recovery. Requests necessary tests and follow-up home visits, and refers patients to specialists, as necessary.

Essential Functions:

* Provides independent primary care, consultation and advice to others as a medical doctor. Makes patient home visits, principally in assisted and independent living facilities, assumes on-call duties, exercise prescriptive authority and directs Interdisciplinary Team Meetings.

* Provide care for patients in a multitude of settings, including home visits, visits to assisted living and independent living facilities, hospitals and nursing homes.

* Acts as the collaborating physician for any nurse practitioners or other advanced practice nurses employed by Kindred at Home/Western Reserve. Serves as a clinical resource to clinical staff. Participates in the education and mentoring of the nursing staff to promote quality of patient care.

* Provides assistance in insuring Kindred at Home/Western Reserve is compliant with the "meaningful use" criteria in its electronic medical record implementation.

* Provide night/weekend call coverage periodically.

* Provide teaching, leadership and other administrative services within Kindred at Home/Western Reserve as reasonably requested.

* Provides appropriate internal and/or external clinical education.

* Attends team meetings, nursing meetings and staff meetings regularly.

* Adheres to practices of confidentiality regarding patients, families, staff and Kindred at Home practices.

* Keep and maintain appropriate records relating to all professional services rendered.

* Must maintain appropriate licensing and continuing education requirements.

Education: Requires a degree in medicine from an accredited medical school. BC/BE in Geriatrics, Internal Medicine, Palliative Care/Hospice or Family Practice.

Licenses/Certification: Active medical license to practice in state which will be working

Experience: A minimum of two years of experience in Geriatrics, Internal Medicine, Palliative Care / Hospice or Family Medicine preferred. Ability to work flexible schedule and travel locally. Meets mandatory continuing education requirements of Kindred at Home and licensing board.
