Florida Hospital seeks to hire a Hospitalist RN who will embrace our mission to extend the healing ministry of Christ.

Department Profile
 
The Hospitalist Service Line crosses all campus at Florida Hospital and is focused on the strategic development and alignment of physician operations with FH system and campus initiatives. Hospitalist are physicians who specialize in providing comprehensive medical care to patients while they are in the hospital. Our hospitalists are based across all of the Florida Hospital campuses and they admit and care for our patients and patients of referring physicians who are receiving in-hospital care, including those who require a hospital stay of less than 24 hours.
 
Work Hours/Shift\:
 
Full time, Monday – Friday, 8 am – 5 pm (possible shift extend until  7 pm and weekend due to census)
 
Job Summary
 
As a Hospitalist RN with System Hospitalist you will make providing service you priority while caring for the whole person in a faith-based atmosphere.
 
The Hospitalist RN functions in a supportive role to the contracted Hospitalist Physicians/Providers. The Hospitalist RN partners with the contracted Hospitalist MD/Provider and the Care Management Team to manage escalated issues and complex cases in an effort to help facilitate patient throughput and appropriate discharge placement. This role actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

  • Bachelor of Science in Nursing
  • Three to five years of experience as a RN
  • Acute care experience (preferred)
  • Experience in Case Management, Managed Care, Discharge Planning and Utilization Management (preferred)
  • Case Management training (preferred)
 
Licensure, Certification or Registration Required\:
 
  • Current valid Florida State Licensure as a RN
  • BLS
  • Case Management certification (American Case Management Association - ACMA/Case Management Society of America - CMSA) (preferred)
 
Job Responsibilities\:
 
Demonstrates, through behavior, Florida Hospital’s core values of Integrity, Compassion, Balance, Excellence, Stewardship, and Teamwork.
 
  • Liaises between Care Management and Hospitalist providers, working collaboratively to solve patient challenges which includes, but not limited to, financial barriers, religious beliefs and cognitive issues. Acts as an escalation team for Floor Care Managers.
  • Follows-up on any outstanding diagnostic test results, pending consults, patient requests or barriers for discharge.  Avoids delay by ensuring test results are available for review and decision making.
  • Connects with Hospitalist providers each day to review discharge plans on all patients.  Identifies patients that have a discharge delay and identifies barriers to discharge. Works with providers to facilitate safe and appropriate discharges. Collaboratively manages interdisciplinary patient discharge plan.  Effectively communicates across continuum of care.
  • Participates in outlier meetings as appropriate.  Communicates information with providers and facilitates changes in plan of care as directed by physician.
  • Shadows all new contracted Hospitalist providers. Performs chart audits and helps identify areas for additional education or information sharing. 
  • Facilitates post discharge communication with Primary Care Physicians and patients as necessary.
  • Uses discretion and adherence to policies and procedures when discussing patient related issues that are confidential in nature. Protects patient privacy with all interactions. Maintains the confidentiality of employees, patients, administrative staff, functions and medical staff information with no infractions.
  • Actively participates collaboratively with the Care Management Department in support of system goals for capacity management.
  • Attends Hospitalist Council or other physician meetings to participate in sharing feedback, present findings and assist with problem identification and resolution. Plays an active role in educating resident physicians and Hospitalists on patient care management from a care management discipline.
  • Maintains knowledge of federal and regulatory policies in the patient care management.
  • Performs other duties as assigned.
