Position:  Registered Nurse (part-time)
Reports to:  Director of Operations
 
Mission Statement: Enhancing the lives of aging adults and their families.
 
 
Hours: Work approximately 8-10 hours per week (work two weekdays per week). NO PHONE CALLS PLEASE.
 
 
Primary Responsibilities:
 
Assessments, Reassessments, Quality Assurance Visits, Training:
 
  • Perform initial and ongoing client assessments of Personal Care clients to determine the appropriate level of service.
  • Initiate, prepare, implement, and evaluate the care plan; make documented revisions to the Plan of Care as necessary.
  • Prepare and maintain clinical documentation including nursing assessments, Plan of Care’s, and discharge summaries for all clients receiving Personal Care.
  • Review and communicate change in the client’s condition or needs to the Director of Operations.
  • Make a supervisory visit to each Personal Care client at least every 30 days to assess the client, the Personal Care employee’s delivery of care, and determine whether client outcomes are being met.  Document findings.
  • Supervise and train Personal Care employees.
 
Secondary Responsibilities:
 
  • Attend periodic training and other company events.
  • Assist with other duties and special projects, as assigned.
