Job Description
This role at Home Instead Senior Care is expected to perform a variety of duties in the area of client services. The LPN will perform Care Consultations in the home as well as Quality Assurance visits, and manage cases.
Primary Responsibilities:
- Develop and maintain positive relationships with all clients through care consultations, quality assurance visits, phone calls and case management
- Monitor, mediate and log both client and CAREGiver activity, and follow up on CAREGiver assignments and client service
- Maintain relationship with client’s family members as needed
- Develop and maintain a relationship with healthcare professionals in the community
- Counsel CAREGivers on quality concerns
- Conduct client/CAREGiver introductions as scheduled
- Enter and maintain accurate client and CAREGivers records in computer
- Work closely with an a team of professionals
Requirements:
- High school graduation or the equivalent; LPN (active license or inactive license)
- Availability to complete some on-call duties
- Must live within a 25 minute drive to the northern portion of Madison County
- Must have a vehicle, current driver’s license, and current automobile insurance.
- Driving record review, criminal background check, and drug screen conducted in hiring process.
