Role of the Home Health RN

Makes initial and on-going skilled nursing visits; provides nursing assessment, treatment and patient/family education as ordered by a physician in a timely manner. Responsible for development of the plan of care under the direction of the physician, updates care plan as needed. We are looking for an RN for primarily home health but will also be cross-trained for hospice. This person and must be willing to travel to care for our patients in Springfield and surrounding areas and will participate in the on-call rotation every 3 to 3 weeks.

Requirements

* 1-2 years experience as an RN, home health or hospice experience preferred

* We require a nursing degree from an accredited school and a current Missouri RN license

* Basic life support training should be completed in 90 days with advanced training required at certain intervals.

Qualifications:

Qualifications

Education: Graduate of an accredited school of nursing.

Licensure: Registered Professional Nurse, currently licensed in Missouri.

Experience: Minimum of one year recent nursing experience.

Certifications: Must have current American Heart Association Heart Saver Provider card or obtain one within the probationary period. Effective 4-1-2010, a new coworker must have a current American Heart Association Healthcare Provider card or obtain one within the probationary period. All Home Care coworkers shall be transitioned to Health Care Provider by June 1, 2011.

Other: Skills, Knowledge, and Abilities: Have comprehensive knowledge of general nursing theory and practice. Ability to work independently. Must be flexible, motivated and have excellent written and oral communication skills.
