As a Transition Coordinator working with the TX STAR KIDS program, you will be providing a vital service through person-centered clinical and transitional services for pediatric members, including case management, assessments, interventions, coaching, resource coordination and transition planning. You may identify, coordinate or assist in resolving highly complex concerns for our pediatric members related to transition planning.
 
This is a field based / telecommuting role that requires approximately 25% - 50% travel within an assigned area in the community including Gainesville, Sherman, Paris, Sulphur Springs and a home based office.
 

 
 
Primary Responsibilities:

Conduct comprehensive member assessment that includes bio-psychosocial, functional and behavioral health needs
Initiate care planning and implementation of a comprehensive care plan which is member-driven and addresses medical and / or behavioral health needs
Engage members routinely telephonically and in-person via home / facility visits to coordinate services, community resources and provide health education
Provide education and information on available benefits and services and how to access appropriate care
Identify early risk factors and targeted condition management interventions as necessary
Create, review and revise Care Plans with a focus on health education, improving health outcomes and adult transition planning
Advocate for member needs and services and provide assistance to members in navigating the healthcare system
Collaborate with the member's individual support systems and other care team members to deliver and coordinate quality care and services
Ensure completion of required touch points and care coordination activities and document all activities within the member clinical record

Requirements


Required Qualifications:


Bachelor's degree or higher plus a minimum of 3 years of  experience working with individuals with disabilities or other specialized populations
Basic level of proficiency in Microsoft Office suite applications (Word, Excel, Outlook / Email, Internet) including the ability to type and talk at the same time and toggle between multiple applications
Reliable transportation and willing to travel throughout surrounding area on regular basis
Access to high speed internet for home office set up
Preferred Qualifications:

Related education, licensure or certification such as BSW, MSW, BCBA, LPC, LCSW, TX SpED teacher certification, etc.
Experience working with pediatric population
Experience with MDCP (Medically Dependent Children’s Program) or CCP (Community Coalition Programs)
Medicare, Medicaid or Medicaid Waiver experience
Bilingual (Spanish)
Understanding of Person Centered Philosophy
Case Management experience
Experience working with school districts and / or ARD committees and Individual Education Plans
