Overview

The QAPI nurse is a registered professional nurse who will develop and coordinate a comprehensive QAPI Plan for Hospice. They will identify strengths and weaknesses by performing chart audits and analyzing data to assure the highest quality of care is being provided for the patients and families. Performs duties and responsibilities in a manner consistent with core values and mission statement

Provide education to staff, hospital nurses, and skilled nursing facilities as requested.

Assures maximum effectiveness of services provided to Mercy Hospice patients by reviewing plan of care and goals - utilizing appropriate criteria, so that patient care is both efficient and effective

Serves as a resource to clinical staff to assure patients admitted to the Hospice program meet eligibility requirements and with other clinical and regulatory questions.

Review staff documentation for appropriateness and complete clinical information and follow up to make sure everything is completed. Work assigned work queues daily.

Determines measurable outcomes. Selects manageable quality indicators. Collects data related to the quality indicators to measure the outcomes. Analyze the data- Benchmark- send report to Risk Management on a quarterly basis.

Coordinate the QAPI program and lead the agency QAPI meetings on a regular basis with the interdisciplinary group.

Submit required data to the Federal regulatory body and other quality vendors as required in a timely fashion. Ensure that the agency is meeting all reporting expectations.

Position Qualifications

Education: Registered Nurse

Licensure: Current RN license in Missouri required.

Experience: Minimum of 2 years hospice experience.

Certifications: Possesses and maintains current CPR certification through the American Heart Association.

Other: Complies with accepted professional standards and practice. Understands Hospice philosophy, principles related to death and dying, and pain management. Has excellent observation, nursing assessment, and communication skills. Has excellent computer skills and is comfortable working in Microsoft Excel and making charts and graphs. Is self motivated and directed with the ability to work with little supervision and meet deadlines. Must be detail oriented when performing chart reviews and comfortable with providing education in a group setting.

Preferred Education: BSN preferred.

Preferred Certifications: CHPN

Preferred Other: Some knowledge of QAPI processes.
