Job Summary/Essential Duties\:

 

 

Principle Duties and Responsibilities

 

Essential Job Functions to be validated at annual review\:

 

Provider\: 

RN I

1.             Utilizes the nursing process to assess and prioritize patient needs.

2.             Formulates, implements and evaluates the patient's plan of care.  Adjusts plan of care in consultation with the RN Team Coordinator as appropriate and implements plan.

3.             Uses basic communication skills to provide care to assigned patients/families.  Relays pertinent information through appropriate documentation and referral to other health care team members such as medicine, dietary and support staff.  Initiates discharge planning working in collaboration with the Case Manager as appropriate.

4.             Initiates appropriate nursing action in crisis or emergency situations.

5.             Carries out physician orders.

6.             Provides continuity of care to meet the patient's changing needs.

7.             Records information on the patient record in accordance with charting guidelines.

8.             Utilizes time management principles and techniques.

9.             Recognizes changes in patient status based on interpretation of assessments.

10.          Recognizes examples of use of evidence/research based practice-at performance appraisal presents journal article supporting a specific approach to a nursing care problem.

RN II

1.             Uses interdisciplinary resources to implement plans of care to promote patient self-care. 

2.             Adapts nursing interventions to promote desired outcomes.

3.             Integrates experience and interdisciplinary expertise to establish therapeutic relationships.

4.             Evaluates outcomes of plans of care with individual/family.

5.             Recognizes examples of use of evidence/research based practice and shares information from nursing journals during staff meetings, practice councils and/or care conferences.

RN III

1.             Uses innovative methods and intuitive skills to implement plans of care which promote self-care for individual/family/group.

2.             Continues to integrate experience and interdisciplinary expertise to establish therapeutic relationships.

3.             Evaluates outcomes of interdisciplinary plans of care with individual/family groups.

4.             Participates in review/revision of policies and procedures incorporating information from current nursing literature/research, or participates in an EBP quality improvement process evaluating patient care outcomes or serves as a member of a research study team.

RN IV

1.             Uses interdisciplinary resources to promote self-care in the plans of care for individual/family/group with complex and unpredictable needs.

2.             Continues to integrate knowledge, skills and experience to establish therapeutic relationships.

3.             Develops innovative methods to implement plans of care.

4.             Serves as mentor in the establishment of therapeutic relationships with individual/family/group.

5.             Develops innovative evaluation methods to measure outcomes for plans of care.

6.             Participates in development of new policies and procedures incorporating information from current nursing literature/research, or introduces practice change or introduces program development, or participates in quality improvement initiatives to evaluate patient care outcomes or serves in a leadership role for a research study.  Is a proponent for changes in clinical practice based on evidence from the literature/research.


 

RN V

1.             Works with CNS/Nurse Educator to assure comprehensive focus on patient care.

2.             Engages in peer practice with health professionals for coordination/collaboration of the total care dynamic.

3.             Assists/guides staff in providing quality nursing care.

4.             Provides expert care to patients.

5.             Nursing practice is patient centered, evidence-based, and outcome oriented.

6.             Provide leadership for integrating evidence-based practice into clinical practice.

7.             Fosters an EBP organizational culture, infrastructure and teamwork.

8.             Participates in nursing research, utilization of research and mentors others in utilization of evidence to shape practice.

 

Teacher\:

RN I

1.      Uses available teaching aids to implement teaching plans of assigned patients and revises the plan based on evaluation of effectiveness.

RN II

1.      Identifies educational needs and implements teaching plan for individual/family.

2.      Selects teaching plans for individual/family.

3.      Evaluates individual/family learning through the use of established criteria and modifies plans.

RN III

1.      Uses interdisciplinary resources to identify educational needs to implement teaching plans for individual/ family/group.

2.      Evaluates individual/family/group learning through the use of established criteria and modifies plans.

RN IV

1.      Collaborates with interdisciplinary resources to implement and evaluate educational needs/learning for individual/family/group.

2.      Serves as a mentor regarding the implementation of teaching plans for individual/family/group.

3.      Provides consultation to colleagues regarding critical analysis modification of teaching plans.

RN V

1.      Disseminates current information/research/EBP to assist clinical team to incorporate this into practice.

2.      Possesses effective communication skills needed to educate patient/nurses/and other health team members.

3.      Willingly serves as both teacher and helper to other nurses.

4.      Organize meetings and educational offerings.

RN I

RN IV

4.      Facilitates policy change or development.

RN V

1.      Focus on assuring quality in patient care and consumer rights and responsibilities in the health care system.

2.      Speaks on individual/family/group behalf when they are unable to do so.

3.      Participates in house-wide quality initiatives and/or Patient Safety Goal implementation.

4.      Evaluates clinical practice guidelines for applicability and feasibility in clinical practice.


Professional/Leader\:

RN I

1.      Demonstrates knowledge about the ethical, legal, professional values of nursing, guided by the American Nurses Association Code of Ethics.

2.      Demonstrates positive communication/problem solving skills.

3.      Assists RN Team Coordinator with coordination of patient care and appropriate utilization of services through use of clinical pathways.

4.      Utilizes time management principles and techniques.

5.      Effectively delegates to all levels of staff in collaboration with RN Team Coordinator based on their scope of practice, job descriptions, abilities and patient/unit needs.

6.      Works to maintain positive team relationships with co-workers, physicians, and other members of the health care team.

RN II

RN III

3.      Collaborates in the assessment, formulation, implementation and evaluation of facility, and unit goals.

4.      Minimum 8 hours continuing education per year beyond hospital mandatory education, 1 of which must be related to research.

5.      Participates in hospital committees, unit project work, preceptor, mentor, team coordinator, national certification, professional organizations or community volunteerism- minimum 3.

RN IV

1.      Contributes to the self-development of others.

3.      Formulates solutions to problems resulting from current issues and trends in nursing practice.

Clinical Skills\:

Qualification:

Education\:

1.     A graduate of an accredited school of nursing (AD, Diploma, Baccalaureate).

2.     Bachelor of Science in Nursing (BSN) required within 5 years of date of hire.

 

Licensure/Certification\:

1.      A valid Registered Nurse license is required.

2.      Valid Basic Life Support (BLS) certification required.

 

Experience/Pre-Requisites\:

RN I

1.      Entry level nursing skills. 

2.      Previous clinical experience desirable, but not essential.

3.      Differentiated Practice advancement is encouraged.

RN II

1.      Equivalent of 2 years current full time RN experience in the acute care setting.

2.      Internal candidates must have completed 1 year of acute care experience and successfully petition to advance to RN II in the Differentiated Practice Model.

3.      External candidate with 2 years full time employment as RN in a hospital or a clinical setting with direct patient care responsibilities can be hired into the RN II level.

RN III

1.      Equivalent of 3 years current full time RN experience in the acute care setting.

2.      Successful petition to advance to RN III in the Differentiated Practice Model.

RN IV

1.      BSN required.

2.      Equivalent of 5 years current full time RN experience in the acute care setting.

3.      Successful petition to advance to RN IV in the Differentiated Practice Model.

RN V

1.      BSN and Masters degree required.

2.      Equivalent of 5 years current full time RN experience in the acute care setting.

3.      Successful petition to advance to RN V in the Differentiated Practice Model.
