  Position: Client Care Coordinator Department: Quality Assurance Reports To: Amanda Schultz  
 
Objective:
The Client Care Coordinator is expected to perform a variety of duties to provide an excellent client focused experience. Quality Assurance visits will require you to collect information, ensure processes are being followed, make suggestions, and provide feedback to supervisors, clients and family members regarding the continued efforts to achieve service excellence. 
Primary Responsibilities:

                     

  • Reflect the values of RRW, Inc., (d.b.a. an independently owned and operated Home Instead Senior Care franchise).
  • Be knowledgable of and follow the program guidelines. 
  • Meet with new referrals within 72 hours of receiving them.
  • Gather and copy all required documents.
  • Assist with obtaining any and all missing documents needed for transition. 
  • Maintain communication with active members while in the transition process.
  • Accompany members, as needed, to DMV, Social Security, Vital Statistics and all other appointments deemed necessary by the  program guidelines. 
  • Maintain active housing searches for all members.  
  • Complete DART applications, as needed, and follow up with the nursing home staff for their portion of it. 
  • Assist members with rental and housing assistance applications.
  • Attend Planning, PT Evaluations & Discharge meetings for every assigned member. 
  • Purchase all required items as instructed by the member’s transition plan.
  • Maintain accurate and up to date information regarding the budget and spending for the transition of each member. 
  • Demonstrate open and effective communication with the franchise owner, colleagues, members, and all other staff involved with the program.
  • Adhere to all company policies, procedures and business ethics codes and ensures that they are communicated and implemented within the team.
  • Perform any and all other functions and responsibilities deemed necessary

Education/Experience Requirements:

  • High school graduation or the equivalent 
  • Must possess a valid driver’s license

 

Knowledge, Skills and Abilities: 

  • Must have an understanding of and uphold the policies and procedures established by RRW, Inc., (d.b.a. an independently owned and operated Home Instead Senior Care franchise) 
  • Must demonstrate excellent oral and written communication skills and the ability to listen effectively 
  • Must have the ability to work independently, maintain confidentiality of information and meet deadlines 
  • Must demonstrate effective interpersonal skills as well as sound judgment and good decision-making skills
  • Must demonstrate discretion, integrity and fair-mindedness consistent with office standards, practices, policies and procedures
  • Must demonstrate knowledge of the senior care industry
  • Must have the ability to organize and prioritize daily, monthly, quarterly and yearly work 
  • Must have the ability to establish good working relationships with the franchise owner, office colleagues, CAREGivers and the community
  • Demonstrate open and effective communication with the franchise owner, colleagues, CAREGivers, clients and family members.
  • Must have the ability to sit at a desk and listen effectively for long periods of time on the telephone
  • Must present a professional appearance and demeanor
  • Must have the ability to operate office equipment
  • Must be patient and congenial on the telephone  
  • Must have computer skills and be proficient in Excel and Word 
  • Must have the availability to work evenings or weekends as required
  • Must have the ability to perform duties in a professional office setting
  • Must have the ability to work as a part of a team
  • Must demonstrate excellent organizational skills
