The Therapy Manager of Home Health is primarily responsible for the management and supervision of certified Physical therapist, Occupational therapist, Speech therapist, COTA and PTA's. Accomplishes physical therapy human resource strategies by determining accountabilities; communicating and enforcing values, policies, and procedures; implementing recruitment, selection, orientation, training, coaching, counseling, disciplinary,  and communication programs; planning, monitoring, appraising, and reviewing job  contributions; planning and reviewing compensation strategies. Assures physical therapy quality services by establishing and maintaining departmental quality assurance program including monitoring of quality indicators, documenting actions taken to resolve problems, provides physical therapy services by evaluating patient care protocols for therapy and measuring outcomes. Required to perform patient therapy evaluations and visits, set up a therapy exercise programs with patient to ensure best patient goal oriented outcomes. Therapy manager will work directly with the home health directors and home health staff to ensure that all therapy patients  are seen per the physician orders and that all orders are followed by the therapist. Will conduct supervisory visits per company policy. Responsible for performing a 10 percent audit on therapy notes to ensure the accuracy of the documentation and remain in compliance with CMS  guidelines and company QA policy. Reports directly to the Administrator / VP of home  health.
Primary Responsibilities:

Manages, supervises and is able to perform all related job duties for  therapist
Works hand and hand with directors of home health to ensure patients are scheduled correctly, orders are scheduled correctly and followed by the  therapist
Capable of performing therapy evaluations and visits when needed
Supervises the therapist for home health on a daily basis to ensure that all job tasks are completed and patients are taken care of before end of  day
Oversees the completion of field staff documentation per company policy
Assists CTC's with guidance of which therapist would be best per patient's needs
Works directly with the home health directors and Clinical Team Managers regarding field staff paperwork, timeliness of completion of  documentation
Assimilates PI indicators and participates in PI meetings
Perform supervisory visits on therapists per company policy
Administer direct patient care in accordance with the written and signed order of the referring physician
Coordinates monthly meetings with department managers to review outstanding orders, discuss concerns and changes within the organization
Advise and consult with the family, inter-disciplinary team members, physicians and other professionals, as appropriate, to ensure optimal patient care
Document all treatments rendered and maintain appropriate work records
Delegate to physical therapist assistant
Reassessment of resource utilization  as patient progresses towards goals
Ensures that therapy evaluations are faxed to MD offices daily to support coordination of care and ensure MD received recommendations that therapist documented
Assist with discharge orders as needed ensuring patients are discharged correctly per MD order
Performs other job duties assigned by manager

Required Qualifications:

Must have earned a graduate degree from an accredited program resulting in a Doctor of Physical Therapy Degree
Board certified
5 + years' experience  as a physical therapist
Excellent written and computer skills
Medical terminology knowledge necessary
3 + years' experience in home health
This position requires Tuberculosis screening as well as proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor’s diagnosis of disease
