Position Overview

The client care coordinator serves as a primary contact for our senior clients and their family members at all stages of the service relationship including pre-service assessments, service plan development, and regular in-home quality assurance visits. The client care coordinator reports directly to the general manager and offers critical insights to improve services for scores of senior clients.  

Skills Required

  • Case Management Skills: Simultaneously tracking care and service plans for scores of clients means candidates must be organized, good at remembering many names, ready to manage a variety of care scenarios from companion care to dementia and end-of-life care. Candidates must have senior care related experience and instincts.
  • Communication Skills: Verbal and written communication is key in respectfully and effectively coordinating efforts of office staff, CAREGivers, clients, and client representatives. This is a leadership position that requires critical thinking and frequent collaboration with team members and management.
  • Sales Skills: Pre-service assessments are really our opportunity to conduct a consultative sales process. You find out what is needed and persuasively position Home Instead as a solution that meets those needs. The expectation is that 85% of your pre-service assessments will result in new clients for Home Instead.

 

Job Requirements

  • One year of related experience in a health care or social services field. Strong preference for geriatric or long-term healthcare experience. 
  • Current Oregon driver's license and an insured vehicle in good repair (you will be reimbursed for mileage).
  • Computer skills a must.  There will be an evaluation on your computer skills as part of the interviewing process. 

 

Location

The desk for this position is in our Tigard office. The job, however, includes many field visits in clients' homes throughout our service area .


Compensation

  • This is a full-time salaried position that will pay between $28,000 per year negotiable based on experience.
