This role will support Nurse Practitioners/Physicians Assistants who provide clinical management and case management for Medicare/Medicaid recipients in the skilled nursing facility.  This function is responsible for Clinical Operations and Medical Management activities across the continuum of care. This includes Case Management, Coordination of care, and Medical Management Consulting. This function may also be responsible for providing Health education, coaching and treatment decision support for members.


 


Primary Responsibilities:



  • Working with a multidisciplinary healthcare team approach in the planning of care and service delivery

  • Engaging members in the nursing home

  • Engaging members and family members telephonically to coordinate services, community resources, and treatment needs

  • Delivering a holistic approach to coordinated care based on the enrollee needs

  • Identifying early risk factors and conducting ongoing assessments and documenting in an electronic file

  • Creating, reviewing and revising Plan of Care documents

  • Functioning as an advocate for members

  • Collaborating with the member's PCP, family and facility to deliver and coordinate necessary services

  • Building relationships with members and their families; assisting them with proper health choices

  • Ensuring cases are documented in a timely manner

  • Building relationships with key stakeholders in the nursing facilities


Requirements

Requirements:



  • High School Diploma/GED

  • Reliable transportation, per 75% travel requirements

  • Ability to cover the assigned territory of  Fulton, Lucas, Ottawa & Wood Counties

  • Active and unrestricted LPN License in the State of Ohio

  • 4+ years of experience as an LPN

  • Intermediate proficiency with software applications, that includes but is not limited to MS Word, Outlook, and other Windows-based applications

  • Excellent Verbal and Communication skills

  • 1+ year experience working in Managed Care 

Assets:



  • Experience providing Care Coordination to persons receiving Long-Term care and/or Home/ Community-based care

  • Experience working in a Field position

  • Working knowledge of Medicaid/ Medicare regulations

  • Experience with Electronic Medical Records
