Case Manager - Skilled Nursing Facility - Full-time / Part-time

The Case Manager will serve as a liaison between the hospital discharge planners Heartland liaisons and Managed Care Organizations to coordinate and obtain prior authorization for patients transitioning to their next level of care.  Responsible for evaluating clinical appropriateness by triaging and reading hospital referrals. This position is part of a centralized office that  focuses on pre-admission authorizations for 28 facilities in Michigan.  In return for your expertise, you'll enjoy excellent training, industry-leading benefits and unlimited opportunities to learn and grow.  Be a part of the team leading the nation in healthcare.
 

Educational Requirements:C.I.R.S (Certified Insurance Rehabilitation Specialist) or C.M.C.M. (Certified Medical Case Manager) preferred; Registered Nurse preferred.Position Requirements:

Minimum three years clinical experience and two years medical case management experience.  Comprehensive knowledge of workers' compensation, insurance, and managed care, required. Ability to negotiate coverage and provide complete and timely case mgmt reports, required. Prior experience with an insurance company, private case mgmt. Company, or HMO, preferred. Familiarity with long- term care and/or sub acute care, useful. Strong oral and written communication skills, required.
