RN or LPN. Case manager is responsible for the coordination and delivery of services to clients enrolled in nurse case management services. Case management services are provided to ensure the physical and mental healthcare needs of our clients are being met. This includes the tracking of healthcare-related appointments,Medication management,Disease process teaching,coordination of care and providing consultation to support programs
Education and Experience: Ky nursing license required. It is preferred that candidates have experience working with the elderly population in community care setting such as Community Case Management, Home Health Care or Discharge planning. One year experience as nurse is required. It is required that the candidates have current CPR/BLS certification, Current driver's license, good driving record, vehicle insurance and adequate transportation.
