About this job

The Regional Director Accountable Care/Health Homes within national Medical & Clinical Operations is responsible for management and administration of Accountable Care Communities program operations and leadership of Health Home implementation within the designated Region. The Regional Director Accountable Care, in partnership with Community & State Regional President, national clinical leadership and local health plan leadership, drives program deployment to achieve affordability savings and programmatic success in multiple states and multiple communities within each state. Within Community & State this position reports to the national Vice President, Accountable Care Programs.

 

Major Responsibilities


  • Provides leadership to and is accountable for the performance and direction of program strategy across multiple states and communities and leads the health plan Accountable Care Communities teams to drive affordability savings at the Community level.

  • Directs local teams to engage and collaborate with community partner hospitals, practices and community agencies to achieve improvement in population health, individual health and reduction in overall costs of care.

Requirements

Requirements:  


  • At least five years of experience leading change initiatives for health care teams in practices/hospital settings

  • Strong clinical data analytic skills measuring process and outcomes in healthcare

  • Strong skills presenting complex data and outcomes to executive teams

  • Medicaid managed care experience

  • Strong process improvement skills/experience

  • Ability to organize and thrive in a fast-paced environment

  • Strong interpersonal skills that enable relationship building

  • Effective oral and written communication skills

 

Additional Assets Preferred:


  • CPHQ certification

  • Six Sigma / Lean certification

  • Master's Degree in Health Informatics
