On site Clinical Nurse Liaison responsible for promotion and utilization of clinical programs with specific focus on increasing engagement in Wellness and Clinical/Disease Management programs.  Nurse must be located in the Louisville, KY area and be able to work onsite at the client on a daily basis.  Our client is Yum! and there is the potential of domestic travel less than 20% including but not limited to Texas and California. 


 


Responsibilities:




  • Responsible for education of members on health and condition management (formal and informal), as well as plan specifics and member resources, in collaboration with OptumHealth


  • Identify and coach chronic conditions


  • Education on healthcare consumerism


  • Identify high risk maternity cases and direct into Healthy Pregnancy Programs


  • Referral of employees to available clinical programs including premium designated physicians


  • Create, manage and implement employer specific programs based on employer needs


  • Knowledge of community resources for referral and community outreach


  • Monitors and analyzes medical management activities; provides support to clinical programs; may perform clinical assessments, biometric screenings and clinical audits as needed


  • Clinical interface with facilities and providers for resolution of issues concerning members, benefit interpretation, program definition and clarification.

Requirements

Requirements:




  • Undergraduate degree or equivalent experience required


  • Current and Unrestricted RN License in state of Kentucky required


  • 3+ years RN experience required


  • 3+ years clinical experience required


  • Computer skills including Word, Excel, and developing PowerPoint presentations


  • Ability to work onsite at client location Monday - Friday during normal business hours



  • Entrepreneurial background to help build program from ground up 


Preferred Assets:




  • Community Outreach experience preferred




  • Wellness / Disease Management program promotion, education and development experience highly preferred 





  • Case Management experience





  • Managed Care experience





  • CCM





  • BS in Nursing or greater highly preferred
