The primary purpose of this position is for meeting all state contract obligations including, but not limited to, timely and comprehensive assessment, timely and comprehensive care plan development, and appropriate follow up for all assigned Skilled Nursing Facility members.


  


Primary Responsibilities include:




  • Engaging members in their homes telephonically to coordinate services, community resources, and  treatment needs



  • Delivering a holistic approach to coordinated care based on the enrollee needs



  • Identifying early risk factors and conducting ongoing assessments and documenting in an electronic file



  • Creating, reviewing and revising Plan of Care plans and focusing on disease management



  • Functioning as an advocate for members and decreasing unnecessary hospitalizations



  • Collaborating with the member's PCP to deliver and coordinate necessary services



  • Building relationships with members and their families; assisting them with proper health choices



  • Ensuring cases are documented in a timely manner

Requirements

Required Qualifications:




  • Current, unrestricted RN license in the State



  • 2+ years experience in a hospital setting, acute care, direct care experience or experience as a telephonic Case Manager for an insurance company



  • Basic level of experience with Microsoft Word, with the ability to navigate a Windows environment


Preferred Qualifications:




  • Bachelor's Degree



  • A background in managed care



  • Case Management experience



  • Certified Case Manager (CCM)



  • Experience/exposure with discharge planning



  • Experience in utilization review, concurrent review and/or risk management



  • Strong organizational skills and multitasking abilities will be keys to success
