This position offers part time employment (29-35hrs per week). C.N.A. or professional medical experience preferred.

Responsibilities include but are not limited to the following:

• Conducting regular home visits.
• Coordinating, assessing, and maintaining care provided.
• Providing accurate and timely documentation of client services reflected in plans of care.
• Planning and scheduling routine client and caregiver supervisory visits.
