We are currently seeking a Patient Care Coordinator for our San Antonio office. The primary function is to provide staff support to the team members, patients, and family members. You will also be maintaining active medical records and be responsible for obtaining Physician signatures in accordance to policy and procedures.   

Primary Responsibilities:
  • Establish, maintain, and close patient medical records according to regulations.
  • Responsible for obtaining all necessary signatures from Physicians, staff, and others as necessary for authorizations, certifications, Doctor's orders, and release of information from Hospice to other parties.
  • Audit medical records in accordance to written criteria including all required documentation, authorizations, and necessary signatures.
  • Purge medical records one year after date of final Discharge.
  • Perform administrative duties related to screening calls and facilitating communication for Clinical Services Managers and other staff between patients, families, and other companies.
  • Keep daily schedule for all team members.
  • Enter Patient Care information into database; Monitor Event tracking including Re-certification dates and provide back up documentation to the Billing Department, as requested.
  • Prepare agenda and attend weekly Team meetings.
  • Provide relief for other Hospice Administrative Support staff.
  • Complete all job duties in a manner that meets the accepted standards of practice and the Optum Pallative and Hospice Care Policies and Procedures.

Requirements

Required Qualifications:
  • High School Diploma/GED.
  • 1+ year of Hospice or Medical/Clinical experience.
  • 6+ months of experience in an Office environment using the telephone and computer as the primary instruments to perform the job duties.
  • Medical Terminology knowledge.
  • EMR or Practice Management System experience.
Preferred Qualifications:  
  • Accounts Payable experience.
  • Intake experience.
  • ALLDocs experience.
